A referral from another physician is not required for a yearly gynecological exam. When you are referred, however, our physicians will send your referring physician a exam report. If you belong to an HMO or managed care insurance program, it is your responsibility to obtain authorization from your Primary Care Physician before your visit.
Please bring your insurance cards with you to your appointments.
We participate with the following insurance carriers: Medicare, Aetna, CIGNA, PPOM, Priority Health, Blue Cross Blue Shield, Blue Care Network, Blue Choice, Blue Cross Blue Shield PPO, ASR, IBA, Humana Tricare, United Health Care PPO, Medicaid, Priority Health Medicaid, and Community Care Medicaid.
As a courtesy, we will bill your insurance carrier, however, all co-insurance and co-payments are due at the time of visit. If you do not have insurance or we do not participate with your plan, you will be responsible for payment at time of service. We are happy to discuss our fees with you in advance. If needed we will work with you to determine a payment schedule.
In order to provide the best and most efficient care for all of our patients appointments must be kept. If you cannot keep your appointment, please call us 24 hours in advance. Failure to show for scheduled appointments could result in dismissal from the practice.
Our physicians reserve the right to dismiss a patient from the practice if the patient is continuously non-compliant, combative or disrespectful to physicians or staff. We also have the right to terminate the physician-patient relationship due to failure to pay for services.
