As consultants, we will work closely with you and your primary care physician. As before, your primary care physician will continue to treat and manage any acute or urgent problems which may arise. If you require a referral from your primary care physician, please bring it with you at the time of your appointment.
Please come 15 minutes before your appointment time in order to complete necessary paperwork.
If your insurance plan requires a referral, this must be presented at the initial visit.
Please bring your current insurance card and method of payment.
Payment: Copays and deductibles are expected at the time of service. We accept cash, check or credit cards: VISA, MasterCard, Discover
For routine calls, please call during office hours. Your call will be returned by a nurse or physician as soon as possible.
It is difficult to treat patients over the phone. Many allergic problems have similar symptoms and require an examination for accurate treatment. It is not good medicine to prescribe medication over the telephone for a problem which has not been evaluated, and therefore we request you make an appointment.
If you are sick, please call as early as possible. It is easier to get an appointment for you that day if you call early in the day.
Calls after regular office hours should be of an emergency nature only. For emergencies, when you cannot reach your primary care physician, please call the office and our 24-hour answering service will get in touch with the physician on call, who will get back to you as soon as possible if the problem cannot wait, go to your nearest emergency room or dial "911."
When coming into the office for injections and/or doctor visits, please remember to always register your name on the Sign-In List at the front desk.
If you must cancel an appointment, please call 24 hours in advance.
Telephone calls for prescriptions will be handled during office hours only. We cannot refill prescriptions after hours or on weekends. You must be a current patient to receive a prescription.
We want our patients to feel free to discuss questions regarding our fees. For your convenience, if you have any questions about our fees, account balances or insurance concerns, we can be reached at (616) 738-4262 during regular business hours. Our biller will return your call as soon as possible.
We request payment at the time of service, unless special arrangements have been made in advance or you belong to a health insurance plan with which we participate (listed below). Deductibles and copays are due at the time each service is rendered. At the time of each visit, you will be given a bill. This is an itemized statement of services rendered, including a diagnosis and the procedures performed during your visit.
